Dungeon Defenders II game Status:
Reasons I've seen this happen:
Golden wyrven - Yes, these horrid things can on-hit a crystal, and if you aren't looking it can often easily get close enough to kill it.
Enemies under the level - On some levels when too many enemies spawn at a time enemies are forced through the ceiling and into the floor. On some levelsthey can even then walk freely and attack stuff. Only defense I know against these is to use auras.
Wyrvens in the walls - Happens a lot on the jungle level. Wyrvens get very close to the crystal in the walls, then jump out when they are almost on the crystal. If you don't have auras up these buggers will get you on Karthaki.
On the other hand, you haven't told me the level(s), so I can only make vague guesses.
Deeper Well, Alchemical Laboratory, Castle Armory in particular.
What defense types do you use? Most of all, what aura do you use and where?

Is your build afkable? If not, can you get someone else to help by dying, then going under the level to watch if enemies are walking under the level?
I can confirm they are going under, saw it happen today during Survival in Alchemy Lab on round 22 - it seems like the first wave of NPCs during that round is spawning way too fast thus shoving part of the first spawn of NPC goblins and warriors under the map. My pet tried attacking them when I stood by the crystal as it was taking damage and aimed for the floor.
I normally don't use auras on that map because my monk isn't high enough yet to have effective auras for Insane mode.
Another bug on that particular map during Survival Mode - Wizards on the two sides will sometimes spawn inside of the walls if too many NPCs are spawning at once. All they do is sit there and summon skeletons that instantly fall over dead, it's rather comical to watch.
