One of the largest quality of life issues remaining in DD is the loot filter. I haven't played DD in a month or more despite already owning the complete Shards pack, and the main reason for that is that I got tired of having to look through every piece of loot since I can't trust the default filter, combined with the filter's huge preference for deleting armor instead of weapons when most players (myself included) would be much better off with more armor drops.
I'm far from the first to suggest this, but the two options we need are 1) To be able to set the ratio of weapons:armor drops we want once we hit the item cap*, and 2) To be able to determine the distribution of stats that determines which items are best, and conversely that determines which items are deleted first. I feel this would be a far better (improvement to the game)/(effort required to make the change) than any other option.
Ideally we would be able to create multiple filters that would allow us to specify the best gear for each of our characters/gear sets, but even a single custom filter would be a huge improvement from the current state of the game.
* This might get a bit complicated with multiplayer games, but the two options that leap to mind are to use the host's preferences for all items, or to divide the item drops into (number of players in the game) groups and let each player's preferences dictate what happens for their drops. You might even do a Diablo 3 type system where any given item in a multiplayer game only appears for one player unless they pick up the item and drop it. Any of those systems, whether it was ideal or not, would be better than the status quo.
